LIFE REVIEW INSTRUCTIONS
1
.Choose an individual to interview who is 65 years old or older and lives independently in the community. The older person may be an acquaintance or a family member but should not reside in your household.
2.If the chosen individual is willing to be interviewed, ask them to sign a consent form for the interview(s) (see last page).  If the person seems reluctant, explain that he or she has a right to refuse and you will select someone else to interview.
3.Use the LIFE REVIEW TOOL below to guide the interview(s).
4.This activity may take more than one interview. At the end of each interview, thank the person for sharing their time and views.  Set up a specific time for the next interview and inform the person of the focus of the next interview. Always inform someone of where you may be reached and when you will return. Do not go to the home of a stranger.
1.In the event an immediate problem is encountered with the interview, contact Dr. Hodges or Ms. Green as soon as possible for assistance.
2.Students are responsible for maintaining confidentiality of the nurse/client relationship. Student behavior during interviews will reflect professionalism at all times. Dress in neat street clothes – follow Troy University’s Dress Policy.
3.Summarize the life story organized according to the LIFE REVIEW TOOL. In a sense, you are writing a biography of the selected individual with an emphasis on his or her feelings about the past and the present.  It is not necessary to include every detail; summarize the conversations (DO NOT simply provide an answer to the questions).  See Life Review Grading Rubric for grading criteria. Upload to Canvas under the Assignments tab & the Life Review Paper icon (see directions for uploading the journal at the end of these instructions).
4.Proper grammar, punctuation, spelling, and use of APA format are expected in all written work. No less than 4 and no more than 10 pages will be accepted (the body of the paper). The Life review is due on the day scheduled. Late assignments will be graded according to the late policy stated in the syllabus.

6.Provide a full citation on a “reference” page of the text book and/or other references used.
