Introduction

The environment of the setting made the patient feel at home and safe. - [ ] Intro - define social participation, social inclusion, health and wellbeing, environment, micro/meso/macro.- [ ] Intro -> How does health outcomes and inequalities affect health and wellbeing RELATED TO YOUR SETTING -> racism/older people/children related to your setting/disabilities . ; explain how you will structure what will be discussed- [ ] Health and wellbeing ->>> exploring occupations

- [ ] Section 3

patient with dementia and falls

is equivalent to scene setting of whats going on in the setting roughly 500 words not too too in depth- [ ] Structure: section 3- [ ] Occupational needs -> whats does the service user want to work on? issue with fall play games or go for a walk, occupational needs of patient - if someone is not able to fulfil Occupational performance then LINK to environment = link to occupational injustice like what are the barriers/facilitators of the environment (below)- [ ] Explore the environment -> what is in the environment Mental health setting, patient with Dementia and fall, older adult, , what type of setting, online? Do people leave the setting?,some patient had leaves for 2 weeks or escort leave- [ ] Explore the service ->> what does the person need to do via OT process, MOHO model?? What models are used?? Is it a clear reasoning, what does the service do, explore their interventions, what is the policy that affects the service, what is the law that affects service, what model of disability is followed, is there a stigma internalised like the patient not wanting to continue treatment etc,

- [ ] Section 4 (a big part of the assignment) : include 3 levels: micro (service user) + meso > How OT impacts the patient if it is a strength or a weakness/ community/MDT team/ local policy specific to trust/ family + macro (laws/ national policies that affect everyone) of the barriers and facilitators, Barriers - e.g -> a person wanting to access online banking but not able to have a phone??, Policy - working together, kids/family, discharge policy, 5yr forward view, Law******- MHA, Equality act, care act (include atleast 1), Regulation - confidentiality,

- [ ] Critical thinking for environment - going in DETAIL, looking at occupational needs of the service user, how that environment the physical one, facilitates/barriers them for them to carry out their occupational needs (distance/support system/space/mobility/adaptations/national policy of not providing equipment due to funding)

- [ ] Section 5 - service user view literature, evidence, interventions, can use policy/legislation to drive some of the intervention, something specific in the intervention and how it links to inclusion participation and promotion for example, (teaching them how to use an app on their phone may lead to them getting a job etc). - [ ] Critical thinking of intervention ON the environment: strengths and weakness; BECAUSE gardening happens IN THAT environment - outside/. older adult had access to the garden in the ward where they were able to do garderning.

Green space = increased participation, in the setting there would be, weakness - it might be outside, if it is raining you have to go inside and participation may decrease. There was park near to the hospital where Occupational thereapist and activity worker use to take patient for walks. Incase if it was raining, the new build hospital had sport hall with OT used to use for patient. - [ ] Another example - Providing equipment on the environment how does that support participation on a wider context. Patient mainly used zimmer frame, walking stick as they were older adult

- [ ] Section 6: suggestions, improvements of the environment (new building had lift which patient were not able to access without a staff member with then), what types of consequences that could happen with the service user and what actions you should take forward in order to minimise potential consequences and improve health and wellbeing. ( all the patient had OT with them whenever they were taken outside).

Improvement-Kitchen- for the kitchen, the new building kitchen didnt have all equipment so the Occuaptional therapist use to take patient across the roaf to the old building to carry out kitchen assessment.) - [ ] If these barriers continue, what could be the potential health outcome? Bad stairs = falls or high table = further back problems - [ ] Actions can include alternatives to intervention - equipment in a small space? Perhaps teach the person a biomechanical approach, navigating the house in order to go the toilet
