Weighting: This assessment is worth 40% of your final course grade.

Word count: 2,000 words

Aim

The OSCA assessment requires students to demonstrate knowledge of the key themes from thecourse with a focus on the clinical assessment skills required to complete a comprehensive mentalhealth assessment and a nursing care plan at a beginning level ability of a registered nurse.This assessment requires students to demonstrate sound clinical mental health assessment andperson-centred care skills which have been developed throughout the entire course. Students will use clinical reasoning to analyse and describe the persons clinical presentation based on theirobservations of a person engaging in a mental health assessment in the video. Students will thendevelop a nursing care plan using evidence-based approaches to deliver safe quality nursing care.Students will refer to scholarly evidence to support the recommendations throughout theassessment.

Assessment Instructions

Access to the OSCA assessment

The OSCA assessment will be completed by students in Week 8 of the course. The OSCA can becarried out at the students home (this assessment does not require attendance on campus as this isa written assessment).

The course coordinator will post the OSCA video link and background documentation to theAssessment 2 page on the learnonline site on Friday 6th September 9am (Week 7). All students willbe able to watch the video as many times as they need to complete the OSCA assessment.Students will also need to read the background documentation related to the consumer to informtheir understanding of the persons situation.

OSCA template

Students must use the OSCA template located on the course site on the Assessment 2 page tocomplete this assessment. Students will be required to follow the template guide, adhere to the wordcount and document the persons situation, background, mental state examination and riskassessment based on your observations of the consumer in the video. Finally, students will developa nursing care plan using evidence-based approaches and justified by NSHQS (2021).

OSCA assessment

To complete this OSCA assessment, you will need to use the OSCA template to address the following criteria in Parts A and B based on the consumer in the video.

Part AConnect with the consumer and watch the video of this person engaging in a mental health assessment and based on your observations, assessment and documentation skills learnt in the course, use the OSCA template to complete:

Introduce the consumer and presenting issues

A summary of the persons presentation including the persons demographic details, NOK, reason forpresenting to health services including change in mental state, signs and symptoms of mental illnessand relevant background information such as history of mental health challenges, history oftreatments and support networks. (Introduction, Situation and Background sections)

Mental state examination

Following the structured Mental State Examination tool within the OSCA template, document yourobservations of the persons mental state taking into account the persons appearance, behaviourduring the interview, thoughts and ideas expressed in the interview, and emotional expression.Provide justification for your assessment findings. Risk assessment

Following the risk assessment categories in the OSCA template, describe the persons risk factorsbased on their presentation and supporting documentation. Describe the persons strengths andways of coping to promote hope and recovery (Assessment section)

Part BBased on the findings of Part A and using the OSCA template, complete a nursing care planinformed by the recovery model which aim to meet the persons unique needs and providejustification for your care plan using high quality evidence and NSHQS principles:

Nursing care plan

Create a nursing care plan focused on the persons mental health needs based on your findingswithin Part A. The care plan should focus on evidence-based strategies which support the person toaddress their symptoms, improve their mental health and actions which aim to promote recovery.(Recommendations section).

Justification for recommendations of care

Using evidence-based research and referring to the topics within the course content, providejustification for your nursing care plan and the nursing approaches which support the personsrecovery. Describe how the NSHQS principles of safety and quality inform the recommendations inthe nursing care plan. (Recommendations section)
