Level of Performance Expected in Assessment Tasks

This is a Certificate IV course and the standard of performance expected is high. We expect that you are able to explain, describe and articulate your response to each assessment task using three to four sentences maximum (approximately 100 words). We do not accept dot pointed answers unless the question asks you to list.

You will not qualify for your Certificate IV WHS until all assessment tasks are completed and you have been deemed competent in all of them.

Self-Paced Training Days

If you feel that after the 5 days of face-to-face training that you would benefit from additional training days, CBD College offers Self-Paced Training Days days throughout the year. No appointment is necessary, and these days run according to the schedule that can be seen in your Student Portal. These days are designed for you to master your learning in any area that you feel you need more training in. These self-paced training days will give you an opportunity to work one-on-one with a WHS trainer and assessor and to come together with WHS students from other cohorts. Assessment questions can also be clarified during these days.

Due Date

The due date is no later than 12 months from the first day of the face-to-face component of your WHS course with CBD College. You will not qualify for your Certificate IV WHS until all assessment tasks are completed and you have been deemed competent in all of them. Please note that your enrolment with CBD College will cease after 12 months and any assessments that CBD College agrees to process due to special needs beyond this date will incur an enrolment fee of $400.

MARKING OF ASSESSMENTS

Requirements for Competency

Assessment tasks will be marked by a qualified assessor. We will endeavour to mark your assessment within 10 business days of submission. To be deemed competent, you will need to have provided evidence that shows competency against all aspects of each unit of competency in the qualification.

Assessment Outcomes

Once marked, CBD College will send you a checklist where you will see your assessors assessment decision of S (Satisfactory) or NYS (Not Yet Satisfactory) marked against each part of your assessment tasks. If you are deemed NYS in any part of the assessment tasks, then the outcome on your checklist will be further evidence required and you will be advised what evidence remains to be submitted. You will be given an opportunity to resubmit your answer for reassessment. You will be issued with your Certificate IV in WHS only after all assessment tasks are deemed S (satisfactory).

Resubmissions

You may be required to re-submit assessment tasks and will be notified in writing via email if this is the case. You will have 2 weeks to then resubmit any assessment tasks initially deemed NYS. After the initial assessment, CBD College will allow a further two resubmissions before a $199 fee will be charged each time a student wishes to be given a further chance to resubmit their work.

Candidates who submit assessment tasks after this period will be deemed not competent unless there is a special need that CBD College are made aware of during this period.

Literacy and numeracy

Candidates will need to have sufficient numeracy and literacy skills to read and comprehend instruction for referencing legislation, preparing reports, completing assessments, and searching the internet.

Please discuss any language, literacy, or numeracy issues with us prior to attending. This will enable the instructor to organise appropriate support facilities, for example we can arrange to conduct assessments orally rather than in the written form, or extra tuition, as appropriate.

ABC Steel Engineering is a steel fabricating company located on the edge of the city. They specialise in the fabrication of steel products for the agricultural, construction and mining sectors as well as for the housing industry. They also make made-to-order pieces of equipment and machinery. They operate from a large purpose-built facility with three factory workshops and employ about 100 workers.

ABC Steel processes include:

Hole punching, cropping & drilling.

Sheet and Plate cutting (laser, plasma, oxy-acetylene, bandsaw, guillotine)

Welding

Light fabrication

Finishing services (galvanising, electroplating, powder coating & architectural paint finishes)

They work with all forms of metal products including beams, columns & lintels, angles, squares & rounds, pipes, SHS (square hollow sections) & RHS (rectangular hollow sections), flat bars & plates, grating & mesh produced from mild steel, stainless steel, and aluminium and are adept at producing high-quality products including large projects (such as hoppers and silos, gantries, platforms, structural steel) or smaller projects (such as security grills, handrails, brackets, ladders, and gates).

Most of the plant and equipment in use at ABC Steel engineering is adequate although some pieces require maintenance. Examples of plant and equipment in use include:

ABC Steel has been in business for several years and in that time, they have seen their company grow both professionally and financially. They have a good rapport with most workers and a lot of them are long-standing employees. They have just taken on 10 immigrant workers who are new to the country. These workers tend to stick to themselves and do not speak much English although their job skills are good, and they are quick to learn by watching others.

One area of the business that needs attention is work health and safety. ABC Steel has had its share of incidents both minor and serious over the years and given the nature and hazards associated with the work it is surprising that injury rates have not been higher.

You have been employed at ABC Steel as a WHS assistant to help assist with the establishment of a WHS management system for the company and to evaluate the overall needs in relation to workplace health and safety.

At this point in time, there is an absence of commitment and budget towards WHS and there is also a lack of understanding of WHS legislation. A WHS policy is in place but has not been integrated into work practices, workplace safe operational procedures are few and far between and injuries and incidents are only recorded if a compensation claim is made. They do have a WHS Manual within their document system, but it is not being utilised and there has never been an audit of their WHS systems. There are no regular WHS meetings, no elected HSRs and no established WHS committee. Evacuation and emergency chemical drills are not practised and there is no health monitoring within the workplace.

The Human Resources Manager, Michelle Baxter, has designed a mentorship program around WHS and Factory 2 Supervisor Norm Smith is a certified WHS trainer and assessor but neither of these functions is being utilised.

As well as helping to rectify the problems outlined above, you will also be required to; assist with effective WHS organisation of chemicals; collect and evaluate workplace hazardous conditions; investigate incidents and file reports on any serious injuries that may occur as well as develop workplace safety alerts.

As WHS assistant, your duties also include helping with producing comprehensive action plans, reports that outline major areas of concern as well as recommendations to help get ABC Steel back on track with its health and safety obligations.

This assessment is about workplace health and safety compliance and deals with government legislation as well as the interpretation of WHS statistics.

Assessment 1 Part 1 WHS Legislation

Contractor services have been engaged by ABC Steel to carry out general metal work duties as work has been very busy and the company are understaffed. The contractors are all South Korean nationals who are fast at their work but have little to no English language skills. They have had a 15-minute induction session that was delivered in English but none of the workers has signed off to state they understood what was discussed. It is worth noting that the company does not carry out regular meetings for WHS and there are no elected HSRs or established WHS committee.

On 12th July 2019 one contractor, Kim Lee was working alone, moving, cutting, and welding a manhole cover inside a silo (confined space). After finishing the weld Kim Lee applies a chemical to clean the piece of metal. He was not wearing safety goggles, a face mask, gloves, or hearing protection. The area was quite dark and noisy and at times temperatures can get above 45 degrees Celsius. He was also working unsupervised as it was lunch time, and the supervisor was having lunch while the worker worked through to make up time on the job task.

While carrying out the finishing task, the chemical fumes have built up in the area and the contractor has collapsed due to toxic fumes. One of the other Korean workers (Jin) who heard the collapse, runs to the scene to help but he has not been trained in first aid. It is relayed through Jin that Kim needs medical help and an ambulance is called.

After Kim has been taken to the hospital a workplace meeting is called by the factory manager to find out what has happened. It is decided that a factory inspection should be carried out to note any other areas where unsafe situations may be occurring.

Head management has asked that the factory managers sit in at a board room meeting to discuss what the companys health and safety legal requirements are and to consider implementing a companywide health and safety management system to help make sure these types of injuries do not occur again in the future. Factory 1 manager has also been informed that this incident will need to be investigated to find out areas of non-compliance and where controls have broken down. You will be on the team as the WHS assistant.

1.4 Describe the legal differences between the WHS Act/Regulations & Codes of Practice. (Min 1 paragraph answer required)

The WHS Act/Regulations & Codes of practice legally differ mainly through enforcement. The Acts & Regulations are passed in parliament and are legally enforced under statute law in court. On the other hand, Codes of Practices are not legally binding, and its role is to outline ways and the steps to safely carry out certain tasks and meet all obligations set out in the WHS Act & Regulations.

In practicality a PCBU should carry out due diligence in using the codes to create Safe Work Methods in order to comply with WHS Acts & Regulations and provide a safe working environment.

1.5 As the WHS assistant you are required to complete this workplace activity to assist your manager in understanding WHS legislative requirements at ABC Steel.

Under the NSW Work Health and Safety Act 2011 and the Work Health Safety Regulations 2017 the duties, rights, and obligations of a person conducting a business or undertaking (PCBU) are outlined. Additionally, the laws and regulations outline the responsibilities of all workers here at ABC Steel.

PCBUs have legal obligations under s19 of the WHS Act 2011 to ensure the safety of workers engaged/workers carrying out activities in the workplace. Also, to ensure the health and safety of other persons is not put at risk from work carried out as part of the conduct of the business or undertaking. PCBU also are responsible to so far as is reasonably practicable, maintain the premises so that the worker occupying the premises is not exposed to risks to health and safety. This may be accomplished at ABC Steel by providing and maintaining a safe work environment, fit-for-purpose plant and buildings, and suitable amenities to assure worker welfare.

Workers & Employees legal obligations are outlined in s27 & s28 of the WHS Act 2011. The act obliges workers to take reasonable care for their own safety as well as the safety of others in the workplace by ensuring their actions do not adversely impact others. Also, they are to follow any instructions given by the PCBU to meet their obligations as set out in the WHS Act 2011.

Instructions to Candidates

Assessment 2 is in two parts and contains 31 questions.

Part one focuses on writing complex documents which include producing a report related to the statistical data in assessment 1, a role-play (meeting management) as well as written questions.

Part two is dedicated to an OHSMS and a WHS policy and requires the completion of a group role play (action planning) as well as written questions.

The assessment involves the scenario - ABC Steel (on pages 6-8)

N.B. Your answers should be at least a paragraph long to show that you have adequately considered these questions. Answers that are only a sentence long are not acceptable.

2.1 Planning Stage. The human resources manager has contacted you via email asking you to:

1) inform management about your statistical findings

2) explain to the ABC Steel workers one issue you identified within your statistical analysis

You will need to first plan which appropriate documents would be required for these two purposes. Referring to the ABC Steel House Style Guide and ABC Steel WHS Manual complete the overview below before you begin to produce your documents.

If there are gaps in your initial findings, describe which additional material or data may be needed to help fill those gaps. Additional information must come from relevant sources and be listed.

At least one graph, diagram or illustration will be included within your final document only (Q2.5b) you may like to plan this during this draft phase.

When you have created the first draft of your text, you must email a copy (in a word document) to your trainer for comments. Assessment 3 is related to the concepts of WHS consultation and training requirements. The assessment contains 22 questions and is in two parts.

Part One is dedicated to Consultation and includes a staff meeting role play as well as written questions and comprehensive reports. Part Two is related to training requirements and includes completion of a training matrix, research activities as well as written questions.

The assessment involves the scenario - ABC Steel (on pages 6 - 8)

Management has asked you to help ABC Steel gain an understanding of their obligations surrounding sharing health and safety information and to initiate changes to meet these obligations.

At this point the facility has a general workplace meeting about once a month in the yard, weather permitting, with all employees invited to attend. This meeting is not generally well attended, and it is often not very health and safety-focused as all business items are discussed. Workers generally do not speak up as it is felt that this meeting is holding up production.

When a health and safety issue is found, it is generally reported to a supervisor who sends the issue to management where it is generally lost, misunderstood, or ignored. Because of this, workers are generally disengaged in talking about health and safety as nothing seems to change.

As a health and safety assistant you will be required to contribute to implementing WHS consultation and participation processes by demonstrating your ability to;

Scenario - Risk Management at ABC Steel.

So far it has been noted that the risk identification at ABC Steel is carried out on an intermittent basis. As we have seen when calculating LTIFR, there is a risk template where injuries and near misses are combined (see document Incidents at ABC Steel.doc). and workers will generally only report an unsafe situation when prompted by a safety supervisor. There is a Hazard Report Form, but it is not being utilised. Your job is to help evaluate current risk management processes, identify appropriate risk management techniques, establish a comprehensive Risk Register that contains a risk analysis component, identify hazardous chemical exposure rates. You are also expected to identify areas of mandatory health monitoring that needs to be undertaken and any wellbeing issues that could be present within the workplace.

Assessment 4 - Part 2 Chemical Safety

ABC Steel has a chemical handling issue!

After a recent toolbox meeting, workers have revealed that there are numerous chemicals used in work processes and there is no safe handling, storage or training associated with the use of hazardous substances. The only control in use is that of disposable dust masks. Workers are all concerned as they feel they have brought this to supervisors attention before, but nothing has been done to rectify the issue.

It was also reported that another worker has been overcome with fumes while cleaning one of the bathroom amenities. He was using a large undiluted amount of bleach and was not wearing gloves or breathing protection. The worker was taken outside and recovered from the effects of dizziness and lack of breathing ability and was then sent to the company GP for a check-up. A painter has also logged a first aid report where she has recorded that she was treated for a skin burn associated with the priming chemical they use before painting.

A supervisor has inspected these areas and noticed there are no storage facilities for any of the chemicals and hazardous substances being used on site. Also, no one is wearing correct PPE, there are no SOPs associated with job tasks and there has been no training or information given to workers. The SDSs associated with these hazardous substances are within the company document system in the admin building but not have distributed within the workplace (The SDSs are provided within your student resources).

Controls for chemical handling have been initiated. Its time to monitor and review that they have been embedded effectively. After talking to workers, they are happy with the new masks and feel a lot safer. Some were unsure about storing all the chemicals in one storage unit. You are a member of the incident investigation team at ABC Steel. The company has a large site with three purpose-built buildings on the outskirts of the city. The company's hours of operation are from Monday to Friday 7 am to 4.30 pm and Saturdays 8 am to 1 pm. There is also scope for overtime when needed usually over the weekend and workers can work up to 60 hours a week when they are busy. There are approximately 100 staff members. These include machinists, cutters, welders, painters, drivers, as well as general labourers. There are times when workers are required to operate offsite, delivering and installing finished steel products.

The workplace policy on rest periods states that staff takes fifteen-minute morning/afternoon breaks and a 30-minute lunch break. This also forms part of the organisations fatigue management plan. This is sometimes overlooked by staff as work is fast-paced and busy.

Employees are reasonably well-paid but work hard for long hours and are under stress to finish jobs to strict time schedules.

On Friday 12th July 2020 at 6.30 pm you were informed that there had been an incident in building 1 and a worker has been taken to Edge Town hospital.

You arrive at building 1 at 6.45 pm. There are about 10 workers who are wandering around. All have stopped work. You immediately secure the site and start notifying stakeholders as per the Incident Investigation Procedure.

Carlos, the forklift driver, leads you to the scene of the incident (next to scaffolding and silo in the middle of building 1 work area) and explains that a welder, Kelvin Jones (47 years old), has slipped from the top of a silo and knocked himself unconscious and he has a minor cut on his leg. That was at 6.00 pm. As Kelvin fell, he grabbed hold of the welding equipment and bottles which fell with him. These were also damaged in the fall and were secured by Steve at the incident site.

There is a labourer who witnessed the incident, (Steve), who called for the assistant manager, Gina, who is also the first aid officer, who administered first aid and called an ambulance (6.03 pm).

By the time the ambulance arrived (6.15 pm) Kelvin had regained consciousness and the cut on his left leg had been treated to stop the bleeding by Gina with wound cleaning wipes, cut saline solution, and wound dressing covered with a crepe bandage. Kelvin was obviously in a lot of pain, going into shock and then taken to hospital (6.25 pm). That is when they called you.

You inspect the work area and notice that fall protection harnesses have not been worn. When you ask Gina why this is so, she states We didnt have time to put the harnesses on as we are too busy, and it was only a five-minute weld.

The following day the return-to-work coordinator has informed you that the injured worker has suffered extensive damage to his head in the fall and will be away from work for two weeks rehabilitating from the effects of the concussion. Interview transcripts - July 17th, 2020

On 5th December 2020, Contractor services had been engaged by ABC Steel to carry out general metalwork duties as work has been very busy and the company are understaffed. The contractors are all South Korean nationals who are fast at their work but have little to no English language skills. They have had a 15-minute induction session that was delivered in English. None of the workers has signed off to state they understood what was discussed in the induction though. James the induction trainer said he believed they understood what he was talking about as they were giving him a thumbs up occasionally.

It is worth noting that the company does not carry out regular meetings for WHS and there are no elected HSRs or established WHS committee. On the day of the incident (05/12/20), the crews in factory 1 started at 0600hrs as they were behind in their job tasks and one job (the silo manhole) in particular had to be finished by the end of the day. They had a quick 5min prestart meeting (0605hrs) to reiterate the need to get the work finished on time.

As mentioned, the task that needed the most attention was a manhole entrance and cover which needed to be cut and fitted to a silo that has been sitting on the back of a truck in factory 2 for the last day. The 6m high silo was brought into the building and placed vertically(1000hrs). The morning tea break siren sounded at 1000hrs. Kim Lee was assigned the task (1005hrs) with one other Korean worker (Jin). The two workers collected their materials for the job from Peter who is the factory 1 supervisor at 1010hrs. Kim Lee entered the silo through a small space (1025hrs) with cutting equipment and began to cut the manhole access. It was noticed by one worker (Sarah) that there was a lot of fumes starting to build up in the area she mentioned her concern to Jin (1155hrs). At 1200hrs the lunch siren sounded. Jin then went to the bathroom.

At 1215hrs, Kim Lee was still working alone, moving, cutting and welding the manhole inside the silo.

After finishing the welding Kim Lee applies a chemical to descale the piece of metal. It has been noted he was not wearing safety goggles, a face mask, gloves or hearing protection. The area was quite dark and noisy and at times temperatures can get above 45 degrees Celsius. He was also working unsupervised as it was lunchtime and the supervisor Peter was having lunch while the worker worked through to make up time on the job task.

While carrying out the finishing task, the chemical fumes have built up in the area and the contractor has collapsed due to toxic fumes. One of the other Korean workers (Jin) who heard the collapse, runs to the scene (1216hrs) to help but he has not been trained in first aid. He waved to Gina the first aid officer to attend. It is relayed to Jin that Kim needs medical help and an ambulance is called (1217hrs). Gina prepared for a confined space entry rescue (entered at 1222hrs) and provided first aid by cleaning Kims face with cold water and placing an oxygen mask over Kims face. She noticed that Kim had sustained a small cut to his forearm which she bandaged with a gauze dressing and antiseptic cream. The ambulance arrived quickly on the scene (1235hrs). Kim was unconscious and the ambulance crew provided first aid (1240hrs). After Kim was then taken to the hospital (1250hrs) and a workplace meeting is called (1300hrs) by the factory manager to find out what has happened. You arrive at the incident site at 12.55 pm. Everyone seems confused as to what to do next. All have stopped work. You immediately take control of the incident as per the requirements of the Incident Investigation Procedure. It is decided that a factory inspection should be carried out (1330hrs) to note any other areas where unsafe situations may be occurring.

On closer inspection certain items are of concern to management and an investigation has been initiated. These concerns include;Signage not in place during operations

Lack of training and induction

Scheduling of work

Supervision of works

It has been relayed to the workplace by hospital staff that Kim had regained consciousness and had suffered a collapse due to restrictions to his breathing. This was most likely caused by a lack of oxygen in the confined space due to overwhelming welding and chemical fumes. He will be recuperating for some time and the return to work officer is notified that Kim is likely to be away from work for at least six weeks.

They are great like that, no questions, just get it done.

We tried to get everyone in early as we needed it finished. The regular guys didnt want to start early though as they had all worked until 10pm the previous night. The Koreans also worked until 10 pm yesterday but they didnt mind coming in early to get some extra overtime. The job was explained to them and they got on with it. They are great like that, no questions, just get it done.

Can you clarify which procedures for this job task were being used by the work crew?Im pretty sure there is a SWMS for welding, not sure if its being used though. We did get the OHS Policy translated into Korean which is on the notice board.

Marking procedure

We will endeavour to mark your assessment tasks within 10 business days of submission.

Your Certificate IV WHS Academic Transcript and Certificate will be available in your Student Portal within 2 business days of being deemed competent.

Resubmissions

You may be required to resubmit certain tasks if your answers are too brief or deficient in some way.

You will be notified by email if you need to resubmit your work.

After the initial assessment, CBD College will allow a further two resubmissions before a $199 fee will be charged each time a student wishes to be given a further chance to resubmit their work.
