In the pharmacy while dispensing the Apixaban 2.5mg, I scanned the wrong strength medicine
Reflection
During my placement in the pharmacy, it was one of my responsibilities to dispense the medicine for the patients. This writing will reflect on challenges, the errored that I made and the success that I have done during my placement. One day when I was dispensing the blood thinner to one of the regular patient. I was not attention to the strength of the medicine. I dispense 5 mg Apixaban instead of 2.5 mg Apixaban .Fortunately after my preceptor after me she noted that mistake , at that time I felt so stress, scared and worried because if the patient takes this medicine he can have bleeding and serious side effect . I learned from this mistake that I have to nocte any mistake as the patient life is my responsibilities as pharmacist.
