Task Purpose
With the increasing prevalence of chronic disease in Australia, adopting a person-centred and best-practice approach to nursing care is essential. This task will enhance your knowledge, skills, and awareness of self-management principles for chronic illness. You will analyse chronic disease models of care, the nurses role in interprofessional collaboration, and development of self-management goals using the selected case study.

Patient Profile Name: Liam
Age: 5
Gender: Male
Grade: Primary school prep year level
Medical History: Cystic Fibrosis (CF)
Social Situation Liam is an only child and lives with his mum Sarah and dad Tom.
Chronic Condition Impacts Liam was diagnosed with cystic fibrosis shortly after birth.
CF is a genetic disorder that primarily affects the respiratory and digestive systems, leading to thick, sticky mucus production in the lungs and digestive tract. Liam experiences recurrent respiratory infections, chronic cough, wheezing, and poor weight gain due to malabsorption of nutrients.
Current Challenges Liams current treatment plan includes airway clearance techniques, nebulised medications, pancreatic enzyme replacement therapy, nutritional supplements, and regular monitoring of lung function.
Liam likes going to school, however due to the unpredictable nature of his condition, he is often required to stay home, and Liam struggles to make and keep friends due to his absence and inability to play most sports like the other children in his age group, impacting his mental health.
